*Company: Twin Cities Physicians*
*Location: Willmar, Minnesota & Surrounding Communities*
*Schedule: Full-Time or Part-Time | Monday–Friday | No Evenings, Weekends, or Hospital Call*
*Compensation: $120,000–$160,000+ Total Compensation* (Base Salary + Productivity Incentive) + *Mileage Reimbursement*
What If Your Next NP Role Gave You a Territory Instead of an Exam Room?
Same clinic. Same hallway. Same exam room.
Patient in. Patient out. Repeat.
There is another way to practice primary care.
Twin Cities Physicians is hiring a *Nurse Practitioner* to provide on-site primary care to older adults throughout *Willmar and surrounding communities*.
Instead of spending every day inside one clinic, you'll build a practice across local senior living communities, bringing medical care directly to patients in *assisted living, memory care, independent living, and skilled nursing settings*.
You'll manage your own patient panel, organize your day, travel between communities, make clinical decisions, and become the provider that residents, families, and senior living teams rely on.
*Your practice isn't confined to four walls.*
Own Your Day. Own Your Patient Panel.
This is a role for an NP who enjoys having *autonomy and responsibility*.
You'll assess, diagnose, treat, prescribe, manage chronic disease, respond to changes in condition, and coordinate care for medically complex older adults.
But you won't be doing it alone.
Twin Cities Physicians is a *physician-led medical group*, giving you clinical and administrative resources while allowing you significant independence in how you manage your patients and territory.
*Independence without isolation.*
And your schedule stays remarkably straightforward:
*Monday through Friday. No evenings. No weekends. No hospital call.*
Your Office Changes. Your Patients Don't.
One morning may begin at an assisted living community. Later, you're rounding at a memory care community a few miles away.
Over time, those aren't simply facilities on your schedule.
They're *your communities.*
You get to know the nursing teams. You understand which patients require closer monitoring. Staff know how you communicate. Families recognize you. You develop relationships that make coordinating care easier.
And because you're seeing patients where they actually live, you gain insight you don't always get during a traditional clinic appointment.
What You'll Be Doing
* Provide *on-site primary care* within senior living communities
* Develop and manage your own panel of older adults
* Diagnose and treat *acute and chronic medical conditions*
* Complete assessments, annual wellness visits, and follow-up care
* Manage medications and chronic disease treatment plans
* Respond to *changes in patient condition*
* Coordinate care following hospitalizations and specialty visits
* Identify concerns early and help *reduce avoidable hospitalizations*
* Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and families
* Educate patients and families
* Support quality, patient satisfaction, and continuity-of-care initiatives
Maybe You're Ready to Get Out of the Clinic
You don't need previous senior living experience to succeed here.
NPs from *family practice, internal medicine, primary care, urgent care, geriatrics, hospital medicine, home health, hospice/palliative care, skilled nursing, and community-based medicine* can bring valuable experience to this model.
The bigger question is whether this style of practice fits you.
You may thrive here if you:
* Enjoy working *independently*
* Want greater ownership over your patient panel
* Like variety instead of spending every day in the same clinical setting
* Are confident making clinical decisions
* Enjoy medically complex patients and chronic disease management
* Build strong relationships with nurses and other healthcare professionals
* Are organized enough to manage a *mobile territory*
* Want meaningful autonomy without opening your own practice
Why NPs Are Making the Move
* *$120,000–$160,000+ total compensation*
* Base salary + *productivity incentive program*
* *Full-Time or Part-Time*
* *Monday through Friday*
* *No evenings, weekends, or hospital call*
* Mileage reimbursement
* Significant *clinical autonomy*
* Your own ongoing patient panel
* Physician-led clinical support
* Comprehensive medical and dental benefits
* *PTO + paid holidays*
* CME allowance
* Malpractice coverage
* DEA reimbursement
* Clinical and administrative resources
What We're Looking For
*Required:*
* Active *Minnesota Nurse Practitioner license*
* Strong assessment, diagnostic, and clinical decision-making skills
* Confidence managing acute and chronic medical conditions
* Strong communication and care coordination skills
* Genuine interest in caring for *older adults*
* Reliable transportation and willingness to travel throughout *Willmar and surrounding communities*
*Especially Relevant Experience:*
* Primary care or family medicine
* Internal medicine
* Geriatrics
* Assisted living or memory care
* Skilled nursing / long-term care
* Home-based primary care
* Home health
* Hospice or palliative care
* Hospital medicine
* Chronic disease management
*Wound Care Certification (WCC)* is a plus.
